TYPES OF MEMBERSHIP
(Complete membership form in folder to hand in with your check.)

Adult Membership (Local):
·      Annual Membership:  $100.00 per year for facilities and custodial services.
·      Quarterly Memberships:  $30.00 per quarter for facilities and custodial services.
·      Non-Member Day Players:  $5.00 per session, not contributing expenses for facilities and custodial services.
 
Other Membership:
·      Annual Membership:   Minors, Students, Members traveling more than 50-miles:  One Half the respective rates shown above for facilities and custodial services.
 
Daily-Play Fees:
·      $2.00 per session for Local Adult Members, both Annual and Quarterly.
·      $5.00 for Non-Member Day Players.
·      $1.00 per session for “Other Membership” players.
 
Special Offer to all Annual and Quarterly Members:
·      Adult Members:  Pay Annual Dues and Daily Playing Fees in advance and  receive a $100 annual discount on Daily Playing Fees, i.e., $1.00 per Daily Playing Fee in lieu of the $2.00 Daily Playing Fee.  That is, play all sessions for one year for the price of $200.00.
·      Other Membership:  Pay Quarterly dues and Daily Playing Fees in advance at the beginning of the quarter and receive a 25% discount of Daily Playing Fees i.e., 26-weeks at $39.00 in lieu of $52.00.


WAIVER
Each player will be required to sign a release/waiver to protect KTTC, Phyllis Wheatley YWCA,
and USATT from liabiilty. 

MEDICAL FORM
Plan to fiill out a medical form for us to keep on file in case of emergency.
It will be on the back of the liability form.
Medical form will be CONFIDENTIAL and for YOUR protection!

*The information you will need to provide includes name, address, ph #, DOB, Contact Person/Ph #, Current medical conditions, Medications/Supplements, Allergies, Doctor's Name/Ph #, Hospital preference, Medical Insurance/Group or ID #.
